This service is provided by a small team of Hospice nurses who care for seriously ill patients in their own homes, working alongside other community services.

Who can be cared for by this service?
Any patient who has advanced progressive illness and is thought to be in the final weeks of life can be referred. These patients are normally already receiving care from a District Nurse and living within the Metropolitan Borough of Bolton.

What help can the service offer?
• Specialist nursing care for the patient
• Support for the patient's family and other carers
• Night nursing
• Daytime respite care which allows carers some time to themselves
