Bolton Hospice Support Services

 

The hospice provides the following support services :

 

  • Planned Day Therapy
  • Outpatient Clinics
  • Therapy Services
  • Carers' Support

 

Planned Day Therapy


Planned Day Therapy offers a range of benefits one day a week to patients:

 

  • A period of assessment and therapy provided by the hospice team, giving carers at home some time to themselves.
  • It gives patients the opportunity to meet other people, get to know the hospice team and ask advice.
  • It can provide fresh approaches to care and support, for example through complementary therapies.
  • It gives an opportunity for patients to talk about their illness and how it is affecting them and their families.

 

We undertake a full nursing assessment and patients are encouraged to participate in planning their care with the nurses. This is regulary reviewed, and following discussions and a further assessment, patients may be discharged.

When is Day Therapy provided?


Patients normally attend one day a week on Monday-Friday, usually between 9am and 4pm. Currently, these sessions are taking place Tuesday-Friday.

 

Where is Day Therapy provided?


We have a purpose-built unit at the hospice with its own entrance. There is vehicle access up to the main door, and easy access to parking.

 

Can Relatives and carers come along?


Relatives and carers are welcome to accompany a patient on their first Day Therapy visit to help them settle. 

 

Carers are welcome to contact our Day Therapy staff on 01204 663066 between 8.30am and 4.00pm to make an appointment to come and speak with staff when they feel they need any help, advice or support.

 

Outpatients

 

Outpatients attend by appointment to see one of the Hospice Consultants or another member of the Hospice team.

 

Attending an outpatient clinic is usually the first opportunity for patients to get to know the Hospice, and for the nursing team to get to know the patient.

 

Can patients be accompanied?


Yes, family members or friends who are close to the patient are very welcome to come along with patients to the clinic. It can be useful for the doctors and nurses to be aware of carers' concerns.

 

Services offered to outpatients:

 

The main reason for an Outpatient visit is to see the hospice Consultant. We can, however, also provide other services
