A number of Social Workers are based
full-time at Bolton Hospice where they form an integral part of the Palliative Care team, carrying out several key roles:
Guiding patients
Our team is there to discuss any concerns patients may have, helping them to make decisions about their care and about the kinds of support they need.
Supporting patients when they leave the Hospice
When a patient is due to be discharged our Social Workers liaise with the Hospice team and also with outside professionals such as the District Nursing Service, Social Services Home Care, Macmillan Nurses and other organisations.

The aim is always to ensure that patients have a safe discharge, and one that suits their wishes and those of their families. This support includes advice on Carers Assessment and Support, Welfare Benefits and also Residential and Nursing Home care for those unable to return home.
Helping people come to terms with a diagnosis
If you or someone close to you has been diagnosed with a terminal illness the changes can be difficult and painful to deal with. Some people manage this on their own, while many others welcome the opportunity to share their feelings and the decisions to be made with someone who understands. Our social workers are extremely experienced in providing this special kind of support.
