Palliative Care Education Passport
The award winning Palliative Care Education Passport was initially developed by Springhill Hospice and has been endorsed by Skills for Care, now the Bolton Hospice education team are excited to be able to deliver this at Bolton Hospice. The programme consists of 6 half-day modules aimed at all roles and disciplines, to provide the knowledge and skills required to care for someone in the last years of their life.
This course is aimed at all roles including nurses, AHP’s, AP’s, and care home staff.
There is also a mini-passport for roles that may not provide personal care or symptom management. Please get in touch with education@boltonhospice.org if you would like to find out more about this option.
Module 1 – Advance Care Planning/recognising advanced disease
Module 2 – The REAL Plan – foundation level communication skills
Module 3 - Psychological and spiritual support for people and their families
Module 4 – Symptom management and assessment
Module 5 – Recognising dying and care at the end of life
Module 6 – Care after death and bereavement support
The cost of completing all 6 modules and achieving the passport is £250. We my be able to offer a group discount if you would like multiple members of staff to complete the passport - please contact the Education Team at education@boltonhospice.org to discuss this!
