Make a Referral
There are several ways a referral can be made. This may be through a healthcare professional, via a GP, through self-referral, or on behalf of a friend or family member. You can self-refer to services at the Wellbeing Hub at Bolton Hospice using the online form below and healthcare professionals can make a referral to clinical services by submitting an e-referral on your GP or alternative system or downloading the form and returning via email.
We can support people with all progressive life-limiting illness such as cancer, COPD, heart disease, and neurological diseases.
We welcome and support everyone irrespective of gender, race, religion or sexual orientation and as an independent charity, all of our services are free of charge
Accessing our Care and Support
What can we help with?
- Difficult pain and symptom control.
- Complex psychosocial problems (patient/carer).
- Need for specialist end of life care and support, which may include bereavement care.
- Need for specialist help with rehabilitation or adjustment to deteriorating function.
- Generalist services are unable to meet the patient or carer needs.
We have a highly skilled team of doctors and nurses who work with other professionals which ensures we deliver a high quality service to meet the individual needs of our patients.
What services do we provide?
Our services are available from the point of diagnosis and we can provide services such as:
- 24 hour advice for carers and healthcare professionals
- Specialist advice sessions and support at our Wellbeing Hub
- Outpatient clinics
- Hospice at Home
- Assessment, rehabilitation and symptom management on our inpatient unit
How does the referral process work?
Once we have received your referral your case will be discussed by our team of doctors and nurses on the same day or the next working day. If we are able to help, we will organise care through the appropriate service, be it Hospice at Home, outpatient clinic, wellbeing hub services or inpatient admission, whichever is best suited to your needs at that time.
Whichever service is recommended to you, you will have access to the multi-disciplinary team which includes our specialist team of doctors and nurses and a number of other professionals such as social workers, physiotherapists, occupational therapists, counselling and spiritual care.
Dad had treatment at the Christie but needed ongoing help with the pain, so his doctor suggested a stay at Bolton Hospice to bring his pain under control so he could continue with his life. Dad had misconceptions about going into a hospice as his view was he would not come back out alive again. However within days he was calling it a 5* all inclusive!
